Parent Handbook Acknowledgment Form

Please read the Parent Handbook and confirm that you have reviewed and understand the information provided by Urban Christian Academy (UCA).

Sign and return the acknowledgment form to the office or email it to info@ucachicago.org. This form must be returned by your first day of childs attendance.
